GRADUATION
REQUIREMENTS

SUCCESS STARTS WITH COMMITMENT.

Our goal is to graduate clients drug and alcohol free and
independently employed, independently housed, and participating
in regular and meaningful groups that support sobriety.

To graduate from Safe Harbor, participants must successfully complete all program requirements listed below.

• Must successfully complete a minimum of 273 days in the Residential phase and 92 days in the Outpatient phase of the program.
• Must obtain Driver License. If you are not able to pay fines in full a payment plan must be established.
• Must have a stable home plan. Weekly rental Motels or similar will not be accepted.
• Must have proof of membership in an outside church or support group such as AA, NA, Celebrate Recovery or similar approved support groups.
• Must have graduated IOP and successfully completed all written and twelve step course work assigned.
• Must have permanent employment.
• Must complete a Personalized Relapse Prevention Plan along with an itemized budget for your living expenses
• Must not have any major infractions within the past 90 days.
• Must pass alcohol and drug screen at time of graduation to graduate. 
• Must Complete and turn in Transition to Phase V Application 45 days before Phase V eligibility date.
• Must begin process of resolving outstanding legal requirements (including fines and fees).
